SCHEDULE:                           Mon Thurs. 8 4:15 PM & Fri. 8 3 PM, Hours may vary based on need

                                                (37.5 hrs/week) 


Under the direction of the Innovative Supports Program Coordinator/Qualified Intellectual Disabilities Professional (QIDP) this position ensures person-centered-services are provided. The Planning Specialist will ensure all service delivery plans (excluding Support Plans & Oversight Plans) under the programs auspices (examples include, but are not limited to IRA Res. Hab, Pre Voc., Day Hab, Respite, Family Member Training) are written in a cohesive manner ensuring services are aligned with one another to reflect the agency's person centered philosophy and approach to service planning and delivery. All members of the Innovative Supports Planning team will observe service delivery and coach staff on the implementation of the services.



  • Participates in the development and authors the person's Habilitation Plan; utilizing valued outcomes from the ISP and input from the person/circle of support.
  • Monitors and ensures the habilitation plan is implemented and documented, as required.
  • Respects the individual's right to privacy and confidentiality by appropriate use of written and verbal communication.
  • Be an active member of each person's planning team.
  • Work collaboratively with others to ensure the individual's health and well-being.
  • Be a Personal Outcome Assessment Interviewer and complete assigned assessments within designated time frame.
  • Actively participate in the development of and achievement of the person's identified personal outcomes.
  • Works closely with service providers, Service/Care Coordinators to ensure effective communication and that habilitation plans are coordinated and consistent. 

MINIMUM QUALIFICATIONS:                                               

  • Associate's Degree in Human Services required


  • One year experience with individuals with developmental disabilities is required. A Bachelor's degree in Human Service or related field can be substituted for years of experience.
