SCHEDULE:  Monday-Thursday: 8am - 4:15pm

                     Friday: 8am - 3pm

                     Some evenings and weekends may be required        

 

JOB SUMMARY:

Under the Director of Community Supports ensures person-centered-services are offered to program participants.  This position also oversees the development and authoring of the habilitation plan for accuracy and for Individualized Service Plan (ISP) meetings.  Additionally, this position supervises Support Specialists and Planning Specialists.  Programs applicable to this position could include:

  • Day Habilitation (On-site and Supplemental)
  • ISS

 

SUMMARY OF RESPONSIBILITIES:

 

  • Supports, coaches, monitors, and provides direct supervision to Support Specialists and Planning Specialists.
  • Identifies and provides needed education and training within assigned program oversight areas
  • Develops and ensures appropriate implementation of Community Supports procedures.
  • Reviews, monitors and takes corrective action to ensure systems and processes are effective within assigned program oversight areas
  • Ensures distribution and accurate interpretation of applicable regulations so that all Community Supports provided by the agency operate within those requirements and adhere to current regulations within assigned program oversight areas.
  • Ensures regulatory and corporate compliance within assigned program oversight areas.
  • Maintains knowledge of applicable regulations so that the programs within assigned oversight areas operate within those requirements
  • Collaborates with the DDRO for service enrollments, regulatory compliance and understanding
  • Uses effective communication skills to establish collaborative relationships
  • Exhibiting professional behavior and relationships
  • Showing respect for diversity and inclusion
  • Organizational participation
  • Contact with families, individuals and other inter Agency and out of Agency service providers.
  • Serves on professional organizations within the field of Developmental Disabilities
  • Under the Director of Community Supports, develops and implements annual budget within assigned oversight areas. Reviews monthly expenditures and variance reports and ensures revisions are incorporated within assigned program oversight areas.
  • Ensures monthly financial management reports develop corrective action to variances and monitor cost effectiveness within assigned program oversight areas.
  • Ensures that billing documentation for services billed through Medicaid, Contracts, and/or other funding entities are prepared monthly and submitted to the Finance office for presentation to Medicaid/and or other funding entity(s) within assigned program oversight areas.
  • Ensures community supports associates are trained and follow Person Centered practices. Applies the principles of Person Centered thinking to service delivery.
  • The person in this position must handle multiple projects in a fast paced and stressful environment. Works independent and/or on a team contingent on departmental/agency needs.
  • Leads departmental projects involving associates at all levels of the division

 

MINIMUM QUALIFICATIONS:                                         

  • Bachelor's degree required in Human Services or related field
  • Two years' supervisory experience and two years' experience in the field of developmental disabilities
